In Med History, our goal is to obtain the best possible medication history for each patient admitted to the hospital. This step is crucial to completion of Joint Commission accreditation-mandated medication reconciliation. Medication History Technicians claim patients from a work list based on the patient’s location and the tech's scheduled shift assignment. Technicians follow a structured interview with the patient or caregiver and document the information into the electronic medical record. Discrepancies are communicated directly to the provider/pharmacist for resolution.
Job SummaryProvides technical assistance in the documentation and clarification of medication history lists under the direction of the pharmacist. Processes, enters, and interprets physician/nurse medication histories, under the direction of the pharmacist, and completes full history documentation in computerized system. Clarifies information through communication with providers, patients, and pharmacies. Performs special projects for the department. Works with multiple medication automation systems.Essential FunctionsProvides professional customer service as a liaison between the pharmacy department and patients/caregivers. Interacts with prescriber as needed.Processes physician/nurse/pharmacist obtained patient histories and enters into the computer system and assists in maintaining patient profiles.Reviews profile for allergies, duplication of drugs, and last doses and brings these to the attention of the pharmacist.Checks and verifies accuracy of patient profiles.Maintains clean and organized work area.Maintains all requirements for CPhT certifications. This position will entail 6 months of On-Site work at Butterworth Hospital
Once fully trained, you may have the opportunity to work Remotely while still residing in Michigan
6 months of full-time equivalent onsite experience required before eligible to work from home.

No scheduled shifts, flexibility to pick up from the open shift list.
Must work a minimum of one shift per 6-week scheduling period to maintain employment.
