JOB SUMMARY
The Medication History Specialist (MHS) improves medication safety for patients by assisting the pharmacist to compile up to date medication lists for patients being admitted to the hospital. Well rounded knowledge of medications is required with the ability to utilize drug information databases to obtain the necessary medication information. The MHS must possess excellent communication skills and problem solving abilities. The ability to handle multiple assignments simultaneously and the ability to work independently as well as part of a team are essential.
JOB QUALIFICATIONS
EDUCATION
For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.
Minimum Required: None
Preferred/Optional: Graduate of an accredited Medical Assistant program or Pharmacy Technician program, or currently enrolled.
EXPERIENCE
Minimum Required: Three years of pharmacy tech experience or five years of Certified Medical Assistant work experience or five years of Certified Nursing Assistant work experience
Preferred/Optional: Motivational interviewing techniques and community pharmacy experience.
CERTIFICATIONS/LICENSES
The following licensure(s), certification(s), registration(s), etc., are required for this position. Licenses with restrictions are subject to review to determine if restrictions are substantially related to the position.
Minimum Required: Certified Pharmacy Technician certification awarded by the Pharmacy Technician Certification Board (PTCB) or Certified Medical Assistant (CMS) certification awarded by the American Association of Medical Assistants (AAMA) or Registered Medical Assistant registration from the American Medical Technologist (AMT) for Medical Assistants within 6 months of hire or Certified Nursing Assistant certification obtained by completing required training courses and successful completion of nurse aide competency examination.
