About the Position:

Organize and sort medication that is delivered from the pharmacy.

Provide support to nursing and provider staff, i.e., print medication forms and medication education sheets, file forms in medical record, fax authorization paperwork, and other administrative functions as needed.

Function as a liaison between program and pharmacy staff, providing consistent communication and support.

Medication Management and Organization Receive medications and sign packing slips for office; fax/email to Life Tree Organize meds in locked cabinets Maintaining logging system for medication sign out Generating/maintaining reporting to identify pertinent focus areas on operational criteria Communication Communicate Discharges, Team Changes, Office Transfers, Regional Census Function as a liaison between the program staff and Life Tree pharmacy in order to facilitate the synergies and quality of care benefits that are offered to patients Email/Fax Face sheets: allergy info, relevant insurance info, etc. Communicate patients in hospital, jail, MIA, etc. Communicate overstocking issues, lost meds, early refill, vacation refills, etc. Be an advocate in office for quality patient care offered by Life Tree, explaining the benefits and advocating for Merakey Organizational synergies realized that optimize patient care and outcomes Clinical Relay clinical questions to nurses/prescribers Relay refill requests to nurses/prescribers and following up Work with nurses/prescribers to complete and submit prior authorizations; follow up with insurance on behalf of prescribers in the interest of patient care Identify patients filling medical meds at other pharmacies and work with team to integrate care in order to keep each patient safe and monitor drug interactions/allergies, etc.  Identify patients that are having LAI filled elsewhere and report on potential outliers of corporate initiative to mitigate risk and prioritize patient safety through utilization of Life Tree for all LAI injection filling. Miscellaneous Ensures compliance with relevant laws, HIPPA regulations and established company policies, procedures, standards and Compliance program. Complies with Life Tree and Merakey policies and procedures. Performs other duties, as assigned, pertinent to position and company expectations. Arrives to work on time, works diligently while not physically/directly supervised, and initiates communication as requested by Life Tree management. At times, when necessary, may play a role in the distribution of medication that may require driving and interacting with patients and/or their responsible parties. Assist Life Tree pharmacy with pharmacy tasks such as, but not limited to: Data entry, adjudication, report generation, and medical records maintenance
