Medication Therapy Management (MTM) Pharmacist
We are excited to announce the launch of our new Medication Therapy Management Program (MTM) at Regional Medical Center. RMC is committed to improving the quality of care for our patients. We are hiring an experienced clinical pharmacist to work with our physicians, patients, and associates to implement a patient centered MTM service. The primary goal of the MTM service is to provide a more comprehensive medication management service that will ensure that patients are receiving the safest and most cost-effective treatment. The MTM program is designed to assist both physicians and patients, by securing safer and more effective medication use through: + Medication Adherence: Improving the quality of patient care and outcomes (e.g., disease/symptom improvement/resolution, improved compliance, saved lives); + Controlling Cost: Reducing health care expenditures (e.g., reduction in expenses associated with physician visits, urgent/emergency care visits and hospitalizations); + •Management Services: Reducing medication-related adverse events (e.g. improved patient safety through lab monitoring and polypharmacy). The ideal candidate would be a graduate of an accredited School of Pharmacy, have completed a PGY1 / PGY2 residency and be registered with the Alabama State Board of Pharmacy. This candidate should also be eligible to become certified through BCPS or NBMTM and practiced for a minimum of three years as a clinical pharmacist in a hospital or ambulatory care setting. Knowledge of the 340B program is highly preferred.
