Description
The COMPASS Mental Health Clinician works as part of an interdisciplinary, patient-centered HIV care team to support the overall health and well-being of people living with HIV/AIDS. The Mental Health Clinician will work to engage and provide ongoing mental health services to patients living with HIV, including clinic-based outreach, program intake, diagnostic assessments, treatment adherence and risk reduction counseling, service plan development, case conferencing and evidence-based psychotherapy interventions. They will also provide crisis intervention and case management as needed.
RESPONSIBILITIES:
+ Provides intakes, screenings, psychosocial assessments, treatment adherence and risk reduction counseling, crisis intervention as well as ongoing therapy to patients living with HIV/AIDS with mental health and/or substance use conditions.
+ Adheres to all grant/programmatic guidelines regarding service delivery and documentation
+ Participates in interdisciplinary case conferences, to support the overall care and ultimately the viral load suppression and retention in care of patients.
+ Participates in the identification, tracking and management of high-risk patients
+ Provides concrete case management as needed, regarding a variety of needs when indicated, including but not limited to: HIV/AIDS Service Administration (HASA), AIDS Drugs Assistance Program (ADAP), transportation assistance, coordination with pharmacies, housing referrals, and completion of M11Q forms
+ Completes risk assessments on patients and assists in making determinations on appropriate level of care; provides safety planning for patients in crisis.
+ Manages caseload proactively, utilizing supervision and available tools to continuously improve care.
+ Completes required documentation in a timely fashion
+ Participates in Continuous Quality Improvement projects to support optimal outcomes for patients.
Qualifications
+ Masters in Social Work from an accredited University required
+ MSW with New York State Certification required
+ NYS licensure as LMSW, LCSW, PhD/PsyD required
+ Strong diagnostic and clinical skills.
+ Willingness to supervise interns and attend required supervisor courses
+ Demonstrated experience working with diverse populations ie: HIV, homeless, mentally ill, elderly, children, adolescents, etc.
+ Healthcare experience preferred.
+ Bi-lingual, Fluent Spanish preferred.
