The Behavioral Health Specialist is responsible for working within a healthcare team in providing direct patient care for patients with chronic mental illness and/or socioeconomic challenges. This individual works within the Patient Centered Medical Home Model in coordination with multiple PCPs and serves as a link to an identified population of patients. Through skilled assessment, education, collaboration and coordination of healthcare and community resources, the Behavioral Health Specialist assists patients to gain self efficacy/management skills, achieve optimum functional health status, and quality of life.

Required Qualifications:
Master's Degree Social Work, or in a Human Services or Psychosocial Mental Health field. 2 years Clinical behavioral health care experience. Advanced clinical experience in behavioral health care including evaluation, diagnosis and intervention of psychiatric and substance use disorders.Preferred Qualifications:
Ph.D. Psychology. Experience with Child & Adolescent Mental Health. Experience working in an integrated collaborative environment. Experience with Electronic Medical Record.Why Join Providence?
