As a Mental Health Hospital Liaison Worker at Change Grow Live, you'll be primarily stationed at the prestigious Highgate Mental Health Centre within the vibrant Camden borough. Based on service need you will also be on occasion, required to support within our Camden based clinic or at other hospitals where Camden CGL has hospital liaison workers stationed. Your role will involve attending the Emergency Department and psychiatric treatment wards where you'll provide rapid access to assessments, brief interventions, and facilitate liaison, discharge planning, and onward referrals for service users who seek hospital care due to substance misuse-related issues.

 

This is an exceptional opportunity, ideally suited for a professional with experience in a related field who is now looking to advance their career. This role will require the successful applicant to support clients on the ward & work in partnership with ward teams and mental health services.

 

The use of psychosocial interventions that are person centred & draw upon clients existing motivation will be required to work in this post as clients are supported through their admission & prepared for community treatment upon discharge.

 

Our Camden service is a vibrant, collaborative team, comprising a dedicated lineup that includes a service manager, consultant psychiatrists, medical professionals, compassionate nurses, attentive healthcare assistants, empathetic recovery staff, passionate volunteers, efficient administrators, and supportive peer mentors.

 

Location: Highgate Mental Health Centre as well as ad hoc requirements to travel to our Camden based drug & alcohol service, or other Camden based hospitals within which we have hospital liaison workers stationed. Camden, Greater London

Hours: Full Time 37.5 per week

Full time Salary: £27,861.26 - £32,002.35 dependent on experience (based on full time working hours, pro rata for part time hours)

Allowance: £4,133.14 Inner London Weighting (based on full time working hours, pro rata for part time hours)

 

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

Responsibilities

About the role:

To develop integrated working within local hospitals which will reduce the impact of repeat hospital admissions for drug and alcohol users.Utilise a range of evidence-based assessment tools to assess levels of problematic alcohol use and develop recovery plans based on suitability for community or inpatient detox.To undertake comprehensive assessments for service users, where appropriate and ensure a wide range of interventions are made available.To refer into testing for Hepatitis A, B, C and HIV and provide post-test counselling for results.Run groups in an inpatient setting on managing cravings & triggers when going on leave from the inpatient setting.To develop referral pathways for individuals with specialist services.Building strong therapeutic relationships with individuals experiencing substance misuse within a hospital setting and fostering effective partnerships with clients to support their unique recovery journeys.Conduct assessments of physical dependency using a symptomatic withdrawal scale, using results to determine safe alcohol detoxification as required.Ensure that procedures are constantly reviewed to promote best practice and evidenced based practice in accordance with UK Guidelines on Clinical Management, NICE guidance, and CGL clinical protocols and procedures.Ensure the smooth transition of service user’s clinical needs into community care with the service.Hours worked will be Monday – Friday, full time. There is a small amount of flexibility available around hours worked. Start & finish times may vary based on hospital requirements & the requirements of the successful applicant & will be mutually agreed upon by Camden CGL, Highgate Mental Health Centre & the successful applicant.

About you:

Knowledge of the issues facing those accessing treatment including appropriate medical interventions, social care and health issues.Experience of managing a case load and achieving positive outcomes for all individualsWork in partnership to formulate release plans which reduce hospital admissionsTraining and/or experience in psychosocial interventionHave the ability to work in an environment where they may be subject to upsetting and disturbing information and patient presentation including behaviour.Have the ability to build working relationships and communicate with all members of the Emergency MDTAbility to manage challenging behaviour in a calm and professional manner
