This position is on the inpatient mental health unit(s) at Mercy/Unity Hospital (Unity Campus). There are inpatient units for both geriatric and adult patients at Unity.Every patient on the inpatient mental health units is assigned a mental health social worker during their hospital stay. The Inpatient Mental Health Social Worker works very closely with the multidisciplinary inpatient care team including the psychiatrist/nurse practitioner, nurse, psychologist, internal medicine provider, clinical therapy group staff, mental health coordinator, pharmacy and additional team members as needed. In this role, the social worker will meet with patients to complete a comprehensive bio-psycho-social assessment, assist with civil commitment/court related paperwork, exams and hearings as needed, coordinate with outpatient supports/providers/care team, and help to develop a comprehensive discharge plan for each patient to ensure they have the support needed for a successful transition back into the community. For those seeking licensure – For full-time employees, we offer 4 hours of Clinical Supervision per month (2 Individual clinical supervision hours and 2 Group clinical supervision hours). This is a full-time, Monday through Friday, day hours position.Job Description:
Provides patients and families with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising family care givers, providing patient education and counseling, and making referrals for other services. May also provide care and case management or interventions designed to promote health, prevent disease, and address barriers to access to healthcare.
