**Responsibilities**
**Essential Key Job Responsibilities:**
1. **Professional Development & Compliance:** Assumes personal responsibility for maintaining professional development, ensuring all required Continuing Education Units (CEUs) are completed, and keeping the license active.
2. **Data Collection & Assessment:** Utilizes knowledge of professional social work to collect and review data from patients, their families, and external sources (e.g., schools, outpatient therapists) to assess the patient’s psychological, social, and discharge needs.
3. **Collaboration & Care Planning:** Records and communicates data to the physician and interdisciplinary treatment team. Collaborates with the physician and treatment team to prioritize the patient's needs and develop an individualized, age-appropriate care plan with specific, measurable goals and interventions. Provides rationale for prioritization and/or deferment of issues.
4. **Therapeutic Interventions:** Demonstrates proficiency and skill in providing therapeutic interventions, including family therapy, individual therapy, and group therapy.
5. **Documentation & Communication:** Maintains accurate documentation of interventions, therapies, and education provided. Instructs patients and/or family members based on their identified needs and the interdisciplinary care plan. Communicates effectively with patients, families, physicians, the treatment team, and other healthcare providers regarding patient needs, including discharge planning and actions.
6. **Ongoing Observation & Reassessment:** Demonstrates effective, continuous observation and reassessment of patients and families, evaluating their response to interventions and any changes in their condition.
7. **Confidentiality & Customer Service:** Maintains patient and family confidentiality, providing sensitive, respectful, and compassionate customer service at all times.
8. **Documentation:** Maintains accurate documentation of interventions, therapies, and education. Ensures all interventions are documented within the required timeframes.
**Qualifications**
**Required Education and Experience:**
+ Master’s Degree in Social Work or a related field is required.
+ A minimum of 4 years of experience in a clinical setting is required.
**Required Licensure and Certifications:**
+ Current Kentucky independent licensure as a Licensed Clinical Social Worker (LCSW), Licensed Professional Clinical Counselor (LPCC), or Licensed Marriage and Family Therapist (LMFT) is required.
+ Basic Life Support (BLS) certification required.
+ Safe Crisis Management (SCM) certification required.
**Pay Range**
$26.38 - $38.26 /hour
