The RN Stroke Navigator will contact the patient before hospital discharge, and then follow the patient after discharge providing coordination between the patient, PCP, neurologist, and other specialists. This position will perform in-hospital and early post-discharge screenings for medication compliance, healthy life style, depression, medical complications, and caregiver burden while being an advocate and anticipating their needs. This increased coordination will benefit patient outcomes and satisfaction, while decreasing unnecessary health care expenditures such as readmission.
