Job Summary:The Neurosciences Nurse Navigator serves as a dedicated resource for patients navigating the complexities of neurologic conditions. This role provides expert clinical support, care coordination, education, and advocacy across the continuum of care, working collaboratively with physicians and interdisciplinary teams to deliver high-quality, patient-centered care. The Navigator supports patients with stroke, spine, neurosurgical, and complex neurological conditions across inpatient and outpatient settings.
Key Responsibilities:Complete comprehensive health assessments addressing physical, psychological, social, and financial factors.
Coordinate and organize timely patient appointments.
Collaborate with physicians and team members to establish evidence-based treatment goals and plans.
Review diagnostics and clinical histories to guide next steps in care.
Monitor care plans, modify as needed, and facilitate interdisciplinary communication.
Identify high-risk patients, provide advocacy, and coordinate supportive resources including end-of-life care when needed.
Educate patients and families on disease processes, treatments, side effects, and available resources.
Support hospital-to-home transitions and work to reduce avoidable readmissions.
Maintain and oversee disease-specific data registries and program metrics.
Participate in quality improvement initiatives and policy development.
Serve as a liaison among patients, providers, and third-party payers.
Assist in neuro-specific initiatives such as MSSIC, Primary Stroke Certifications, and collaborative partnerships across Trinity Health regions.
Perform discharge calls for spine and neurosurgical patients.
Engage in community outreach and support groups.
Neurosciences Subspecialty Focus:Collaborate with stroke coordinator on data abstraction, stroke committees, and policy development.
Facilitate smooth care transitions for complex neurological cases with Grand Rapids navigators.
Participate in and support performance metrics aligned with steering committee objectives.
Minimum Qualifications:Graduate of an accredited nursing program.
Bachelor’s degree in Nursing (BSN) required; Master’s degree preferred or in progress.
Current RN license in the State of Michigan.
Minimum 5 years of clinical RN experience, with recent case management or neurology experience strongly preferred.
At least 1 year of experience within Trinity Health preferred.
Strong critical thinking, care coordination, and patient education skills required.
