14803BRDepartment Name:HSP NICU 7N Location:Meriter Hospital Job Description: The role of the NICU Family Liaison and Outcomes Coordinator is to assist the care team (provider, nurse, other members of the NICU patient care team) by coordinating inpatient and discharge care needs. The NICU Family Liaison and Outcomes Coordinator will coordinate services and provide advocacy and referral services to patients and their families to address their medical and psychosocial needs throughout the neonatal continuum of care.

Qualifications:
+ BSN
+ 2-3 years nursing experience as NICU level III nurse
+ Current licensure as a Registered Nurse in the state of Wisconsin Current
Preferred Qualifications:
+ Previous care coordination, patient advocate or family advisory experience
+ Certification in specialty (NICU)
+ Familiarity with EPIC documentation system
