YOUR CONTRIBUTION TO OUR SUCCESS!A Non-Medical Care Manager contributes to AHF by Providing coordination, guidance, and assistance in improving access to and retention of needed medical support to mitigate and eliminate barriers to HIV care services. With a caring and knowledgeable focus, you will work with our providers and healthcare center team so that every Ryan White client receives the support they require and access to available resources. Completing Initial Assessments and developing Individualized Care Plans related to psychosocial needs and goals. You will draw on your experience and skills in understanding basic needs such as housing, food, and clothing, develop relationships with programs and agencies serving those needs, and facilitate referrals, support, and assistance for your clients who lack those services. In this way, you will support the client’s ability to better understand their health status so that they can work to achieve their healthcare goals.
A Non-Medical Care Manager enhances the reputation of AHF by utilizing your case management experience, you will meet our clients where they are, identify strengths and opportunities for growth, and assist them with realistic goal setting. With your skills of collaboration and engagement, you will bring the essence of the Ryan White Care Act - the safety net for uninsured HIV/AIDS patients - to our most vulnerable population. Your needs assessments, care plans, referrals, and support will directly contribute to AHF’s success in meeting the deliverables of our Ryan White contract. Beyond the scope of the contract, your work contributes to measures established by the HIV/AIDS Bureau (HAB) Measures and the Department of Health and Human Services.

AHF has a collaborative organizational structure where staff are accountable to multiple leaders.
The Non-Medical Care Manager will work as a part of a close-knit team to collaborate with the provider and client to implement the optimum care plan for these non-medical needs by improving access to care and retention. Advocate on behalf of clients to decrease service gaps and remove barriers to service. Work with the client each step towards viral suppression, better health outcomes, and quality of life.
Licenses and Certifications: Associate Degree/Bachelor degree in health or human services-related field preferred.
A minimum of 2 years of experience working with persons living with HIV may be substituted for the bachelor.
High school diploma or General Education degree (GED) required.
