Essential Duties: + Completes and documents a discharge planning assesment on identified patients. + Facilitates the development of a multidisciplinary discharge plan. + Oversees and evaluates the dishcharge plan. + Ensures timely and smooth transitions to the appropriate type and setting of post-acute services for the patient's needs. + Identifies risk for readmission and implements interventions to mitigate risks. Shift: 6am - 4:30pm Schedule: Tuesday - Friday **Qualifications** License/Certifications: + State of Oregon RN License + American Heart Association BLS for Health Care Providers within 30 days of hire + Certified Case Manager (CCM), Accredited Case Manager (ACM-RN) or UM Certification preferred Experience: + 2 years acute hospital experience. A Master's degree in Nursing or Case Management may be substituted for 1 year of experience. + 5 years of nursing experience preferred + Case Management experience preferred Education: + Bachelor's degree in nursing or a healthcare field preferred **Pay Range** $51.17 - $71.49 /hour We are an equal opportunity/affirmative action employer.
