· Provide education and support to clients with complex medical conditions, facilitating their wellness and prevention initiatives
· Facilitate health education groups and wellness initiatives, promoting healthy lifestyle choices
· Participate in treatment plan development and implementation, ensuring comprehensive care
· Collaborate with treatment team members to identify and address client needsEducation, Experience, and/or Credential Qualifications:· Active RN license
· Experience with the IDD population preferredAdditional Qualifications:· Excellent written and verbal communication skills
· Ability to implement new innovative systems
· Ability to make decisions based on critical thinking and nursing process
· Successful completion of background check, including criminal record, driving record, abuse/neglect, and fingerprint check
· Current driver's license, acceptable driving record, and current auto insurancePhysical Requirements:· ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally (exists up the 1/3 of the time) and/or a negligible amount of force frequently (exists 1/3 to 2/3 of the time) to lift, carry, push, or pull, or otherwise move objects, including the human body.
· Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift.
