Key Responsibilities:Personalized Care Planning: Collaborate with interdisciplinary teams to design and implement tailored treatment plans that address unique client needs and goals.Wellness Program Development: Create and launch initiatives that promote healthy habits, disease prevention, and overall well-being for clients.Health Education: Lead interactive groups and workshops that empower clients to manage complex medical conditions, make informed decisions, and adopt healthy lifestyles.Care Coordination: Oversee hospital admissions and discharges, ensuring seamless transitions and continuity of care for Healthcare Home patients.Patient Empowerment: Educate clients on their medical diagnoses, treatment options, and medication management, enabling them to take an active role in their healthcare journey.Technology Integration: Implement and leverage innovative technology programs, such as CyberAccess and metabolic screening, to enhance client care and outcomes.Performance Monitoring: Track, analyze, and report key performance measures, using data-driven insights to inform care decisions and drive quality improvement.Effective Communication: Demonstrate exceptional verbal and written communication skills, actively listening to clients, and communicating complex information in a clear, concise manner.Critical Thinking: Apply analytical skills and nursing expertise to make informed decisions, solve problems, and drive positive outcomes in client care.

Requirements:Active RN or LPN licenseDirect patient care experience preferredStrong understanding of chronic disease processesExcellent communication and interpersonal skillsAbility to adapt to new technologiesCritical thinking and problem-solving skillsPassion for exceptional patient care
