Job Title Nurse Case Manager on-call/UHC Requisition Number DU12978 Department Name H4019: Continuum of Care Work Location Lexington, KY Salary Range $26.00-45.70/hour Type of Position Staff Position Time Status On Call Required Education BSN Required Related Experience 3 years Required License/Registration/Certification Registered Nurse Physical Requirements This position requires intermittently sitting at a computer workstation for extended periods of time and performing tasks with repetitive motions (such as typing); and occasionally standing or walking with objects weighing up to 25 pounds; and lifting, pushing, and/or pulling objects weighing up to 50 pounds. Also involves some exposure to loud noises, temperature extremes, hazardous chemicals and fumes including waste, cuts/punctures, blood-borne/airborne pathogens, and combative/violent people. Shift Must be able to work at least 36 hours per month from varying shifts including days/evenings/nights and can include some weekend and or holiday requirements. Job Summary The RN Case Manager is responsible for the clinical aspect of care transition/discharge planning. This includes rapport building/communication with multidisciplinary teams, patients, families and significant others. Discharge planning should begin day of admission and include needed resources/communication for a smooth transition from one setting to the next. Discharge planning should be thorough to prevent 30-day readmissions and should assure high levels of patient/family satisfaction. The RN Case Manager holds responsibilities to the patient/family and multidisciplinary teams to communicate completely and concisely in a timely manner to assure all members are fully aware of next steps regarding care transition.
