**Req Number** 140622 FlexStaff is seeking a Nurse Case Manager/Staff Nurse to join our client's team, a leading organization that provides housing, services and programs for individuals with intellectual and developmental disabilities. This is a full-time (Mon-Fri), direct hire position, there are various group home locations in BROOKLYN. *Hours can be flexible 8-4pm, 9-5pm or 7am-3pm. Responsibilities: + Coordinate the delivery of health care services for adults with disabilities residing in residential programs. + Be responsible for performing nursing assessments and diagnoses + Work in conjunction with other members of the interdisciplinary team to achieve positive health outcomes. + Provides guidance and training to direct care staff to ensure proper delivery of health care services. Requirements include: + NYS RN License + Prior nursing experience and/or experience working with persons with developmental disabilities preferred. + Excellent oral and written communication skills. + Ability to travel to and access multiple locations throughout the metropolitan area. + Able to work flexible hours and/or days.
