CVP is seeking RN Case Manager to join our team in providing our Nation's Veterans with healthcare services as part of the Referral Coordination Team at the Rocky Mountain Regional VA Medical Center in Aurora, CO. This is an on-site position with working hours Monday-Friday, 7:30 AM - 4:30 PM.

 

Location: Rocky Mountain Regional VA Medical Center (RMRVAMC), 1700 N. Wheeling Street, Aurora, CO 80045

 

Duration: 5/1/25- 4/30/26

 

Pay: $29.98-52.35/hr (Depending on Experience)

+

Health & Welfare: $4.93/hr (Can receive as cash in lieu of benefits)

Responsibilities Maintain patient/resident confidentiality and a safe patient/resident care environment, including familiarity with requirements such as medication administration, patient/resident/family education, discharge process, restraint usage, hazardous waste disposal, etc., as defined by The Joint Commission and all other VA regulatory bodies (i.e. Long Term Care Institute and Office of Inspector General).Triage specialty consults for clinical appropriateness and completeness within the National RCI Standards for coordinated careCommunicate with veterans on their healthcare options via phone calls to determine receiving care at VA facility or through community eligible providersHelp veterans make informed decisions on how/when/where to receive specialty care if they are eligible for community careProvide clinical knowledge to veterans and act as a clinical resource to multidisciplinary staff (MDs, LIPs, SW, HAS, etc.) within VISN19 and outside the VISNCollaborate with CACs to rebuild/modify consult templatesServe as a liaison to both internal staff and community care staff during the consult processBuild trusting relationships with specialty providers in clinics where RCI is implementedAttend twice-weekly team huddles and team training sessions as requiredProcess individual view alerts in CPRS within seven days of creationDemonstrate Integrity, Commitment, Advocacy, Respect, and Excellence (ICARE) values in all interactionsProvide patient/resident-centered care by performing the following tasks:Assess the health care needs of the patient/resident and maintain or develop a plan of care to meet interdisciplinary goalsAssess and act on significant changes in the patient’s/resident’s health condition.Carry out the plan of care by performing/assigning delegated tasksCollect and document pertinent data into the electronic patient/resident medical record using designated temp plates with no personalized or customized documentation.Complete nursing assessments within required time frames, and in accordance with policy and procedureIdentify and act on predictable needs of the patient/residentMeet the health care needs of patient/resident by assessing and assisting the patient/resident to meet expressed needsPrioritize and complete tasks within the RN’s assigned shiftUpdate patient/resident care plansNotify assigned physician of any changes in condition.Practice the VA core values Integrity, Commitment, Advocacy, Respect, and excellence ( ICARE ).Report incidents of verbal or physical abuse immediately to the Nurse Manager of the unit they are assigned; or designated off-shift nursing leadership.Coordinate and participate in admission and discharge of patientRespond and assist in an emergency situationPPE and Hand Hygiene ComplianceClear all CPRS clinical alerts daily. Qualifications US Citizenship required to undergo Government sponsored background investigation and credentialingApplicants are required to have resided in the U.S. for the last 3 years.Graduate of a school of professional nursing approved by the appropriate State agency and accredited by the Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE)Current, full, active and unrestricted license as a Registered Nurse in at least one state, territory, or commonwealth in the U.S. or the District of Columbia, demonstrated by providing a copy of current licensure and applicable statesMinimum of two (2) years of case management experience within the last three (3) yearsCurrent Basic Life Support (BLS) certification requiredStrong computer skills and ability to learn VA electronic health record systemsExperience with referral management and care coordination Ability to communicate effectively both verbally and in writingKnowledge of medical terminology and clinical documentation standards
