Responsible for providing comprehensive nursing services to women and their families eligible for the Nurse Family Partnership program in Ocean County.  

Responsibilities Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workersAttends weekly team meetings, case conferences and clinical supervision meetingsAssesses and evaluates needs of women and their families as they relate to health and life course developmentCarries a caseload of up to 25 clients and completes home visitation in accordance with the NFP model elements and NFP Visit-to-Visit guidelinesProvides counseling and instruction to assist women and their families to attain targeted goalsDevelops and maintains community relationships to support client referrals Precepts and mentors new hires and makes visits with student nurses as neededAdheres to agency infection control and safety policiesAdheres to the organization’s policy in regards to absenteeism and appearance Qualifications Bachelor’s degree in Nursing 2-3 years work related experienceStrong computer skills  Excellent verbal and written communication skills Excellent interpersonal and organizational skillsBilingual- Spanish speaking a plus Options Apply for this job onlineApplyShareEmail This Job To A FriendRefer Sorry the Share function is not working properly at this moment.
