Job Summary:
The Nurse Manager will operate within the Healthcare Homes program and serve as a resource to Nurse Care Managers as well as completing hospitalization follow-up calls for our clients. The Nurse Manager will be proficient in providing disease management education to an at-risk population and coordinate care among multiple providers.
Essential Job Functions:
Develop wellness and prevention initiatives.
Cooperatively develop a health goal and update treatment plan post hospitalization as needed.
Coordinate with Community Support Specialists and assigned Nurse Care Manager regarding additional client needs post hospitalization
Manage the process of hospital admission and discharge for Healthcare Home patients.
Train patients on complex health conditions regarding diagnosis, treatments and medications.
Implement, as required, DMH Net Health technology programs such as Cyberaccess and CareManager
Act as a resource for Nurse Care Managers for complex client needs
Functions in accordance with Brightli Operational Guidelines, and within his/her professional code of ethics.
Accesses emergency services in order to provide rapid aid in case of emotional crisis.
Participates in required meetings.
Regularly participates in evaluation of self and assigned staff according to Brightli Operational Guidelines.
Is responsive to all Quality Assurance procedures of Brightli, cooperates with utilization review procedures and follows established procedures, rules and regulations of the Clinical Records department.
Performs other special functions that may, from time to time, be outlined and attached hereto.
Knowledge, Skills, and Abilities:
Ability to make decisions based on critical thinking and nursing process
Excellent written and verbal communication.
Experience and Education Qualifications:
Advanced knowledge acquired through the completion of an RN or LPN license.
Experience with direct contact patient care preferred.
Knowledge of chronic health conditions in both the youth and adult population
Lead or Charge Nurse experience preferred.
Supervisory Requirements:

Provide leadership and guidance to all aspects of the department.
Take an active role in monitoring the identification, development, and execution of strategic objectives.
Involve, as appropriate, all team members to achieve goals.
Effectively communicate to team members any changes and newsworthy events within the department or company.
Handle difficult team member situations directly, using appropriate discretion and Human Resource advice to show respect for the individual
Champion change and effectively manage the implementation of new ideas.
Reinforces team approach throughout functions; support and solicit input from team members at all levels within the company.
Employment Requirements:
Successful completion of background check including criminal record, driving record, abuse/neglect and fingerprint check.
Completion of New Hire Orientation at the beginning of employment.
All training requirements including Relias at the beginning of employment and annually thereafter.
Current driver’s license, acceptable driving record and current auto insurance.
Physical Requirements:

Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift.
Sedentary work involves sitting most of the time but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met.
