Overview Will be responsible for to provide assistance to patients, their families and caregivers to help overcome barriers to care, whether through the health care system or the environment, and facilitate timely access to quality medical and psychosocial care.  Will serve as patient advocate and facilitate transition to the next phase of care. 

Will facilitate access for patients/physicians, ensure patient entry into system at most appropriate point, work to

reduce outmigration due to perception of delays or lack of services and ensure patients referred outside remain in system for other available services.

RN or LPN preferred, will consider CMA or RN student with oncology experience
