Full Time, 1st Shift

Description:

Serves as a clinical resource, patient advocate, and system navigator for cancer care and serves as a liaison between the organization, patients, families, and the community for cancer groupsProvides written and verbal breast cancer information related to the patient’s diagnosis, pathology plan of treatment, and survival plan of careFacilitates information gathering and analysis for patients to make informed decisionsAssesses patient’s educational and physio/psychosocial needs and barriers to careIndividualizes the care of breast cancer patients throughout the continuum of careCollaborates with multidisciplinary team members to facilitate timely access and continuity of care across various care settingsWorks with healthcare providers in maintaining concordance with evidenced-based care and with established and adopted guidelinesEnhances self-development on a continual basis by maintaining current knowledge in clinical education, reviewing professional literature and participating in professional organizations

Requirements:

Bachelor of Science in Nursing (BSN)Minimum of two years of relevant work experienceCurrent KY state license as a registered nurseBasic life supportOncology nursing experience preferred but not requiredMust obtain Certification in Oncology Nursing (OCN, AOCN, AOCNS or AOCNP) within 1 year of hire
