Position Summary:

The Care Coordinator RN is a professional nurse who collaborates with physicians, health care team members, and patient/families to coordinate care for specific patient populations across the continuum and between care settings during an episode of illness.  The Care Coordinator RN participates in the assessment, plan, monitoring, and evaluation of the ongoing care of assigned patient group as they transition through the continuum of care. They are responsible for coordinating the delivery of resources for effective, efficient, and appropriate patient care during an entire episode of illness.  The Care Coordinator is accountable for ongoing monitoring and evaluation of patient outcomes.

What you will do:
• Works in collaboration with their respective providers, APPs, and fellow care team members to provide concise, efficient, and coordinated care to the cardiac patients requiring procedural intervention.
• Helps educate patients on their cardiac condition and any upcoming procedures.
• Works closely with surgery and procedural scheduling to ensure timely care for the cardiac patient.
• Helps create educational materials for conferences.
• Maintains and updates registries specific to procedural services and works to ensure fulfilment of CON quality measures.
• Effectively communicates insurance verification requirements and works with referral staff to ensure proper prior authorization is obtained for any upcoming procedures.

What you will need:

1.    Graduate of an accredited school of nursing.  Bachelor of Science degree in Nursing required Master’s degree in nursing (or actively pursuing master’s degree in nursing) preferred. 

2.    Current licensure to practice nursing in Michigan.

3.    One year of experience at Trinity Health and two years of clinical experience in nursing.

4.    Customer-focused interpersonal skills in order to interact in an effective manner with physicians, health team members, community agencies, and patients and families.

5.    Leadership qualities including time management, verbal and written communication skills, listening skills, problem solving, decision-making, priority setting, work delegation and work organization.

6.    Knowledge or wiliness to learn regulatory and accrediting agency standards and regulations relative to utilization management.  Knowledge of federal, state and local regulations affecting the utilization review program and payment as well as current knowledge of third-party payor programs, requirements and criteria utilized. (For utilization review nurses only)
