Nurse Practitioner—Cardiovascular Administration 

Looking for a new and exciting opportunity?  We are looking for a Nurse Practitioner to assist with coordinating services for our Heart Failure and Structural Heart patients.

SCHEDULE: SCHEDULE:   Full Time for 40 hrs./ week. No on-call requirements

HEART AND VASCULAR APP SERVICE HIGHLIGHTS:

The H&V APRN provides specialized care for patients with heart failure and structural heart conditions. This role involves comprehensive assessment, treatment, and management of patients with complex cardiovascular conditions. The APRN works closely with cardiologists, surgeons, and other healthcare professionals to deliver high-quality, patient centered care. The APRN will help drive key quality metrics in both the inpatient and outpatient setting.

APP values diversity and encourages creative perspectives to improve patient satisfaction and key quality metrics. Monitor patients’ progress, adjusting treatment plans as necessary to optimize outcomes.

QUALIFICATIONS:

Bachelor of Nursing (BSN) Required.  Master’s degree of Nurse Practitioner Program (NP) required. Acute Care hospital setting experience required. 2 years as a Nurse Practitioner (NP) strongly preferred. ACNP strongly preferred. Cardiovascular experience preferred. Registered Nurse (RN) required. Advanced Practice Nurse (APRN) required. Nurse Practitioner (NP) required. Basic Cardiac Life Support (BCLS) prior to providing independent patient care and maintained quarterly Required. Advanced Cardiac Life Support (ACLS) within 30 days of hire preferred.

WHAT YOU WILL DO:

Provide patient education on heart failure management, medications, lifestyle modifications, and post procedural care. Collaborate with other healthcare providers to coordinate care and ensure continuity of treatment. Participate in quality improvement initiatives and help drive key performance indicators (KPIs). Maintain accurate and up to date medical records in compliance with regulatory standards. Work in both the inpatient and outpatient setting
