The nurse practitioner will be responsible for the delivery of medical care to patients primarily with CKD and/or ESRD/ESKD. Primary duties include completing an annual risk assessment visit, creating strategies to improve or manage a patient health status, provide support to the Somatus Care Team for patients with complex needs, and assist with quality care gap closure. Responsibilities Conducts annual risk assessments visits in the patient’s home and or via telehealth Counsels and educates patients and families about benefits and programs available to help them live healthier lives. Documents a complete history which includes a medical plan with appropriate clinical recommendations. Responsible for the coordination of care with Somatus Careteam, primary care providers, specialists and appropriate ancillary services. Completes all documentation and paperwork in a timely manner. Maintains quality of care standards as defined by the practice. This position will not be office-based but will be in the state in which employed and will need to attend periodic training/meetings outside of that state. Facilitates closing gaps in care by educating patients about preventive, monitoring and working with Somatus Careteam and physician practices to schedule diagnostic testing. Participates in the integrated care team meetings. Knowledge of disease diagnosis, prevention, and up to date clinical guidelines. Participates in care coordination with the Careteam Other duties as assigned. Qualifications Required Active and unrestricted Registered Nurse and Nurse Practitioner license. Board certified by ANCC or AANP. Current CPR card by American Heart Association or American Red Cross At least 2 to 3 years of experience in primary care or family practice managing chronic conditions Ability to work without direct supervision and practice autonomously. Reliable transportation and private, secure office space to conduct telehealth visits. Must be proficient with medical instruments and equipment required for the work. Knowledge of computer-based data management programs and information systems, as well as medical records and point-of-interview technology. Ability to communicate effectively, in verbal and written form, with retail and medical partners at various levels, patients, family members, physicians and representatives of the community. Sound understanding of all federal and state regulations including HIPAA and OSHA. Preferred Managed Care/IPA/Health Plan experience. Knowledge of HCC and recapturing chronic diagnoses. Nephrology experience is highly desired. Experience conducting annual risk assessment visits or similar comprehensive visits in the home.
