At Your Door: Visiting Healthcare Services is hiring a Nurse Practitioner/Physician Assistant to work in Columbus, OH area! Our visiting provider group serves Assisted Living, Independent Living, and Skilled Nursing Facilities. If you enjoy autonomy, working with geriatrics, and controlling your schedule, this position may be for you!
This is a full-time, W2 position with benefits and PTO! We are looking for a provider that can dedicate 4-5 days a week. On-call for your facilities Monday through Thursday until 9pm and occasional shared weekend on-call rotation (on-call consists of phone calls only). Increased salary also available with productivity and benefits to be discussed upon interview.
PURPOSE
The Nurse Practitioner is responsible for providing nursing care services to residents/clients as assigned, making observations on an advanced level, forming nursing judgments, carrying out nursing actions based upon understanding of the scientific principles involved, reporting and recording observations appropriately and supervising staff.
Qualified candidates for this position offer:
Education: Graduate of an accredited school of nursing with a Master’s Degree in Nursing and a nurse practitioner certification.Experience: Minimum of two years as an RN in a health care setting.Licensure: Current Ohio RN License and Nurse Practitioner in good standing.ESSENTIAL FUNCTIONS
Answer patient care calls from partnering AYD senior living facilities ranging from patient status changes, to medication changes, prescription refill requests, patient falls, sentinel events and urgent patient/family concernsActs as consultant to facility clinical staff including on advising them when appropriate work up and report out is necessary to most effectively identify what is neededDocuments all patient concerns in Athena, AYD EMRDifferentiates between different types of facilities (SNF/AL/IL) & inquiring on various levels of scopes of practice within the facility to know which measures are appropriate to handle within facility or the need for send out to a higher level of care or more prompt evaluation, with the end goal being to treat the patient in place whenever appropriate. Manages abnormal diagnostic testing & lab results & schedules lab follow ups when appropriate Brief admission reviews: generic chart set up, reviewing medications & sending any scheduled medications to facility specific pharmacy when residents are sent without hard scriptsApproves Release of Body orders on Non-hospice status patients, providing facility with correct collaborating physician info for Provider covering the facility ( Google spreadsheet available), ensuring family, funeral home & coroner have all been contacted by the facility & documenting Time of Death to assist collaborating physician in signing of death certificate. * if patient is hospice status, they will handle all above, but a notification call is usually givenConsults for family members/IL residents who call looking for direction on medical decisionsprovides excellent customer service, prompt return phone calls, and assists facility staff/patients/families in after care needsCompleted other functions such as chart reviews such as risk adjusting during times not responding to callsOther duties as neededIn return, National Church Residences offers an excellent total reward package that includes:
