Job Duties

Demonstrates competence in brief assessment, crisis management, triage, diagnosis,

and treatment within a brief solution-focused model.

Completes assessment for new patients admitted to the service, including history and physical, medication reconciliation, problem list and plan of care.

Performs assessment and plans for ongoing treatment for patients in the service.

Provides psychiatric medication monitoring and counseling as necessary to assigned patients.

Maintains an active presence on primary care clinical teams. Participates in daily

medical team hurdles, medical staff meetings, and interdisciplinary teams meetings.

Assists the primary care provider in recognizing, treating, and managing mental

health and psychosocial issues and acts as a contributing member to the primary

care team.

Accepts “warm handoffs” from primary care teams, and consults with primary care

providers in real time to assist patients with psychosocial issues.

Actively collaborates with primary care providers on patients needs, treatment

planning, and progress in managing behavioral health conditions.

Proactively meets with patients within the primary care setting as an active member

of the primary care team.

Plans a responsible for follow up for patients with abnormal test result and intervenes with patient according to medical center policy and procedure.

Adheres to all medical center policies and procedures and bylaws of the medical staff.

Participates in department performance improvement initiatives, collects and aggregates data as needed, works with leadership to develop a plan of correction.

Fosters and promotes teamwork with other staff members of the service as well as providers from other agencies involved with the patient’s care.

Communicates pertinent information to the supervisor of the service, collaborating physician, treatment team and other community providers as necessary and in adherence to all standards of privacy and confidentiality.

Performs comprehensive physical, psychosocial, and mental examinations, including developmental and functional status determination.

Documents in the patient’s record all pertinent patient interactions and care.

Provides discharge instructions with prescriptions given and follow-up visits arranged.

Participates in interdisciplinary quality improvement efforts to improve clinical practice.

Customer Service: respect, flexibility, knowledge, confidence, professionalism, pleasant attitude, patience and helpfulness. All responses should be timely, professional, caring, and respectful in accordance with Customer Service Performance expectations.

Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental, and infection control standards. Assesses gaps in policies and procedures and create necessary policies and procedures to fulfill these gaps.

Understands and adheres to the Medical Center’s Code of Conduct.

Familiar with the Medical Center’s Mission, Vision, and Values Statements.

 

OTHER JOB DUTIES

Participates in continuing education activities; attends scheduled in-service programs; shares information gained at conferences with colleagues.

Performs other related duties as required.

 

BASIC COMPETENCIES

Education:

Graduation from an approved Nurse Practitioner program.

Current NJ licensure and certification as a Nurse Practitioner in Psychiatry.

BLS certification required.

Holds current certification for prescriptive privileges.

 

Experience:

2 years of related clinical nursing experience preferred.

 

Skills:

Professional knowledge of nursing theory and practice, extensive knowledge of physical assessment, differential diagnosis, pathophysiology, pharmacology, psychiatry and behavioral health.

Good oral and written communication skills.

Good interpersonal skills.

Speaks, reads and writes English to the extent required by the position.
