Nurse Practitioner - Executive Health Concierge

Highlights

The Executive Health Concierge will manage all aspects of the assigned program area (i.e. Executive and Physician Physical Program). This dynamic role combines advanced practice provider clinical duties with the leadership of the Executive Health Program's operations. This includes the program's creation, implementation, and expansion. This position requires a highly skilled and motivated individual who is passionate about providing exceptional care and driving program excellence. The Executive Health Concierge will work collaboratively with medical professionals, administrative staff, and external partners to ensure the highest standards of patient care and program development.

LICENSE & CERTIFICATION REQUIREMENTS

4+ years of Advanced Practice Provider Experience Must successfully complete a nationally recognized Advanced Practice Nurse certification examination within days of graduation or have current national certification. Must maintain certification on an ongoing basis. Must be eligible for prescriptive authority under the criteria outlined by the Indiana State Board of Nursing, Title 848, IAC 5-1-1. Must maintain eligibility for prescriptive authority on an ongoing basis.

ESSENTIAL FUNCTIONS (JOB DUTIES)

Executive Health Concierge: Discusses clinical activities that occur during an executive physical. Schedules executive physical activities with prospective patient and work with prospective patient’s staff to arrange details. Schedules executive physical activities across specialties within CHNw. These may include dietary, cardiology, integrative medicine and others. Works along side CHNw internal stake holders such as marketing, IT, finance, foundation, patient satisfaction, building services and other departments. Conducts follow up video visits with patients to discuss results. Handle incoming and make outbound calls for program-related inquiries. Reviews patients’ journey at a programmatic level and alter for scalability and patient preference. Collects, review and analyze data for decision making. Organizes and report monthly/quarterly program performance meetings related to specific metrics. Collaboration: Plans therapeutic interventions jointly with the patient and Physician with the ultimate goal of developing problem solving and self-care abilities of the patient to the greatest possible extent. The diagnosis gives direction to the plan of care, although the plan may include actions needed to further delineate (or diagnose) the problem. Works directly with the collaborating physician(s) and in coordination with the local management to establish the necessary scheduling templates within the EHR in order to meet patient care needs. Medications: Appropriately prescribes and supervises medications and the necessary education on medication adherence as required for patient care in collaboration with patient’s physician. Responsible for the maintenance of documentation of the required 5% prescriptive reviews completed by the NP’s collaborating physician. Accountabilities: Attends and participated in staff meetings, in-services, projects, committee and community service opportunities as requested or assigned. Adheres and promotes adherence to all organizational and network Policies and Procedures. Will provide care to patients at other additional practices or sites not considered as NP’s home location as requested. Shares in the collaboration of practice quality numbers to be reviewed at least quarterly, so as to understand the impact to the total practice. Initiates and participate in the development and maintenance of quality improvement systems and processes within their home practice and across the organization. Uses technology and appropriate software as an aid to management in planning, implementation and monitoring of care, presenting and communicating information. Manages and assesses risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients. Reports risk issues to the Compliance Officer. Patient Care: Providing age and culturally appropriate care, assesses the health status, illness conditions, response to illness and health risks of individuals, families, and groups, employing the skills of taking histories, conducting physical examinations, and using laboratory and other diagnostic data. May also assess resources, strengths and weaknesses, coping behaviors impact of environment upon the patient. Identifies normal and abnormal findings in history and physical exam. Orders appropriate laboratory and x-ray studies. Diagnoses the actual or potential health problem and need, based upon analysis of the data collected. Intervenes to assist patient, increasing the patient’s participation in his or her own care to the greatest possible extent. Types of intervention include measures to promote health, protect against disease, treat and manage acute and chronic illness, and limit disability by providing prompt treatment. Interventions may include, but are not limited to, direct nursing care, prescriptions or medications and other therapies, and consultation with or referral to other health care providers and/or community agencies. As a health care team member, nurse practitioners have the responsibility for coordination of care that involves other health professionals or resources. Evaluates with the patient-and when indicated, with the collaborating health care provider or team-the effectiveness, comprehensiveness, and continuity of the intervention through assessment, modification of the plan, if necessary, and documentation of health care outcomes. Proactively and positively contributes to increasing the satisfaction level of all patients at the practice, including positively involving the staff in improvement efforts. Individuals may not perform all of these duties, or may perform additional, related duties not listed here.
