Nurse Practitioner for Interstitial Lung Disease

Are you living your life's work? **Summary:** We are seeking a dedicated and skilled Advanced Practice Provider (APP) to join our multidisciplinary Interstitial Lung Disease (ILD) team. The ILD team is comprised of physicians, nurses, social workers, and patient service staff, and works collaboratively to provide comprehensive care to patients with complex and often progressive lung conditions. The APP will play a vital role in managing these patients by performing assessments, developing care plans, providing education, and supporting the ILD team in delivering high-quality, compassionate care. As an APP on the ILD team, you will have the opportunity to make a direct impact on patient outcomes, work closely with experts in pulmonology, and help shape the future of care for this complex patient population. You will also have a role in developing and implementing clinical protocols as well as leadership opportunities. **Responsibilities:** + **Patient Care** : + Perform comprehensive histories and physical exams for patients with interstitial lung disease. + Develop and manage individualized treatment plans in collaboration with the ILD care team, including physicians, nurses, and other multidisciplinary team members. + Write and manage orders for medications, diagnostic tests, and other interventions in accordance with the APP's credentialing and scope of practice. + Participate in regular ILD team meetings to review patient progress and discuss complex cases, ensuring continuity of care. + Provide direct care and follow-up for ILD patients in the outpatient settings as needed. + **Patient and Family Education** : + Educate patients and their families about the nature of interstitial lung disease, treatment options, symptom management, and lifestyle modifications. + Provide support and guidance in navigating the complexities of living with a chronic, progressive lung condition. + Ensure patients understand and can adhere to prescribed treatments and interventions. + **Collaboration and Teamwork** : + Work closely with the ILD team to develop and implement care strategies, ensuring that the patient’s needs are met holistically. + Engage in ongoing communication with the multidisciplinary team, coordinating care and addressing any concerns related to patient management. + Participate in case conferences, team meetings, and continuing education opportunities to enhance clinical knowledge and improve team dynamics. **Credentials:** + BLS/CPR, as a healthcare provider as per the American Heart Association (For providers in the ambulatory setting) required. + Certified Nurse PractitionerPA (Required) + Prescriptive Authority (Required) + Registered Nurse - PA (Required) **Education or Equivalent Experience:** + Master of Arts or Science (Required) + Education Specialization:Master's Degree or Doctor of Nursing Practice (DNP) in NursingEquivalent Experience:•And New to practice or < 2.5 years, The CRNP must be reviewed by the Credentials Committee and approved by the Medical Board.
