Nurse Practitioner - Full-Time
Richmond Community Services is a leading non-profit organization providing support to individuals with developmental, intellectual, and behavioral challenges in Westchester County. We are currently seeking experienced Nurse Practitioners with a background in developmental disabilities to join our team. Position Overview: Under the direction of the Medical Director, the Nurse Practitioner will be responsible for developing and implementing medical care plans for individuals served, in collaboration with the Individual Treatment Team (ITT) and the Individual Program Plan (IPP). Key Responsibilities: + Conduct physical exams annually and as needed, including basic screenings for vision and hearing, and document findings of these exams. + Write medical orders and prescriptions for medications, devices, and immunizations in accordance with established medical protocols and clinical practices. + Communicate with individuals’ parents/guardians via phone, email, or written correspondence regarding medical emergencies or significant changes in medical condition. + Evaluate, initiate, and monitor individualized habilitation programs in collaboration with rehabilitation staff. + Document all pertinent information in progress notes and review related documents regularly. + Coordinate with other healthcare professionals at acute care facilities or clinics regarding individuals' medical status. + Conduct initial history and physical exams for new admissions, documenting findings and making recommendations. + Provide updates to Team Leaders regarding medical information or changes in the status of individuals served. + Review relevant medical history and perform necessary assessments of individuals’ health. + Order and evaluate diagnostic testing (lab work, special studies) as needed based on the individual's medical condition. + Request consultations and evaluate recommendations from specialists, issuing orders based on their advice and clinical protocols. + Make diagnoses and develop appropriate treatment plans in line with Nurse Practitioner standards. + Monitor and adjust seizure control medication, including blood serum levels where appropriate. + Perform discharge physical exams and document findings, preparing written recommendations for new placements. + May be assigned other tasks and duties at the discretion of the Medical Director. Qualifications: + Valid Nurse Practitioner license with experience in developmental disabilities or related fields. + Strong communication skills, with the ability to collaborate effectively with a multi-disciplinary team. + Knowledge of clinical protocols and medical best practices for individuals with developmental, intellectual, and behavioral challenges. Educational and Certification Requirements: + Graduate of an accredited professional nursing program, with a preference for a concentration in Family Practice, Pediatrics, or Rehabilitation Nursing. + Current Nurse Practitioner (NP) license in New York State, with certification through ANCC or AANP. + Must hold valid certifications in CPR, Infection Control, and DEA. Experience Requirements: + At least one year of experience working with individuals with developmental disabilities. + Proficiency in Electronic Medical Records (EMR) systems and e-prescribing platforms.
