Minimum Qualifications:
-Active Licensure and certification as a Nurse Practitioner in either Family or Adult.
-Experience in quality-based care preferred.
-Experience in chronic care management.
-Experience with Transitional Care Management preferred.
-Experience with provider education and patient education.
-NP should have good customer service as well as written, verbal, and interpersonal skills.
-NP should be able to work independently and with a multidisciplinary team.
-NP should exhibit critical thinking skills; great time management, organizational skills, quick learning, and EHR knowledge.
Minimum Requirements:
-Understanding of healthcare operations and systems; advanced knowledge of Excel, PowerPoint, and Microsoft Word.
-Able to function/work effectively within an ever-changing environment.
-Able to meet deadlines and reprioritize as necessary; navigate pertinent software, systems and databases utilized by the healthcare system; and assist with projects to meet deadlines and expectations.
-Able to create data-based and user friendly feedback as well as remediation plans.
Responsibilities:
-The Population Health Nurse Practitioner will be responsible for helping enroll new members to CCM.
-NP will complete care plans for all newly enrolled CCM members and update/revise any existing care plans as needed.
-NP will be responsible for helping with TCM services as needed and appropriate.
-NP will assist with AWVs when needed.
-NP will help with medication adherence for all Medicare and Advantage Plan members.
-NP will help with quality metrics uploading for all payer sites when needed.
-NP will help educate providers and staff regarding quality based care.
-NP will help educate patients when appropriate.
-When healthcare system starts a Chronic Care Clinic, the NP will see and treat any enrolled members until all quality -metrics are met and all chronic conditions are stable and controlled.
-NP should be able to assess, diagnose, and treat patients with multiple chronic conditions.
-Collaborate with Director of Population Health and other staff to help the healthcare system and providers achieve, master, and obtain excellent or above average quality scores.
