Call us at 812-238-4574 or email us at mstearley@union.health Nurse Practitioner details: Position Details: + Full time + 40 hours per week + M-F 8:00am – 5pm + Primary role is: + Provide comprehensive care to patients within the clinic and inpatient setting postpartum + Create a smooth transition to postpartum care for women needing added medical management both postpartum and beyond Essential Job Duties: + The APP will see a risk-stratified population of postpartum patients who experience HTN, DM, obesity, and other chronic conditions. + Completes the routine physical assessment using common assessment skills in a systematic manner. + Orders, interprets, and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs. + Records physical findings, formulates plan and prognosis, based on patient's condition. + Performs physical examinations and preventive health measures within prescribed guidelines and instructions of physician. + Respects patients' rights, including the right to inform, privacy, confidentiality, and the right to refuse care. + Assists with patient education to promote self-care and patient independence. + Follows company policies and procedures and department protocols.
