Park Nicollet Clinic is seeking a certified Nurse Practitioner or Physician Assistant to join its innovative Smart Care clinic in St. Louis Park. Deliver comprehensive services in health promotion, screening, diagnosis and disease management through alternative delivery methods such as e-visits, telephone visits, and video visits with fewer in-person traditional office visits.  Provide team-based care with other highly skilled physicians, advanced practice clinicians and nurses who will practice to the fullest extent of their training and licensure.  Previous experience working as a nurse practitioner or physician assistant in primary care is required.

Flexible scheduling options. Part-time or full-time from .6FTE to 1.0FTE.
