Job Location Duke University Hospital Job Summary

The Geriatrics service at Duke is actively seeking an Advanced Practice Provider (PA/NP) to support the Inpatient service at Duke University Hospital

 

Job Duties and Responsibilities:

Inpatient (approximately 70% of clinical effort)

Perform daily inpatient rounding, consults, and follow-ups (approximately 70% of work duties) Perform physical exams, order diagnostics, and prescribe treatments. Coordinate care transitions from acute care to SNFs, including medication reviews, risk stratification, and family/patient education. Address physical, psychological, and environmental factors impacting successful rehabilitation. Collaborate with Geriatrics and Palliative Care teams, ensuring safe transitions. Communicate with SNF providers at discharge and provide follow-up within 72 hours. Participate in geriatric workgroups, NICHE (Nurses Improving Care for Health system Elders) initiatives, and strategic planning.

Outpatient (approximately 30% of clinical effort)

See clinic patient in Duke South including new and follow up visits Round on skilled nursing facility patients Respond to patient queries via telephone

 

Job Eligibility Requirements

Education

NP:         Graduate of DNP or MSN Nurse Practitioner program (ANP, FNP, ACNP all accepted).

PA:         Graduate of ARC accredited Master’s Degree or Doctoral Degree Physician Assistant program

 

Certification 

NP:         NP certification (ie: ANP, AGNP, AGPCNP, ACNP, AGACNP, FNP)

PA:         PA-C

 

Licensure  

NP:         Current unrestricted RN license issued by or recognized by the NCBON with authorization to practice as an NP.

PA:         Current unrestricted medical license issued by NCMB with authorization to practice as a PA.

 

Experience

One year APP experience is preferred but not required. Geriatric and/or transitional care experience is preferred

 

Other

Must have clean driving record and access to reliable transportation (mileage reimbursement is available)

 

Physical and mental requirements:

Able to clearly articulate scope of practice, and practices within those guidelines as a Nurse Practitioner / Physician Assistant in the state of North Carolina and within the Duke University Health System.  Able to independently seek out resources and work collaboratively to solve complex problems Able to communicate clearly with patients, families, visitors, healthcare team, physicians, administrators, leadership, and others. Able to use sensory and cognitive functions to process and prioritize information, perform health assessments, treatment, and follow-up. Able to use fine motor skills Able to record activities, document assessments & interventions; prepare reports and presentations; Able to use computer and learn new software programs Able to provide leadership in clinical area of expertise and in meeting organizational goals Able to navigate the Hospital and DUHS to provide clinical expertise to specific patient populations Able to withstand prolonged standing and walking with the ability to move or lift at least fifty pounds Able to remain focused and organized Able to work collaboratively with all levels of personnel

 

Job Hours

FOUR ten-hour shifts per week. Hours are 7am-5pm. No weekend or on-call coverage requirements.  Schedule may be changed by administrative or clinical leadership to meet department needs.
