Job Summary

The Geisinger Skilled Nursing Program is seeking an experienced Nurse Practitioner or Physician Assistant for the Embassy of Wyoming Valley, Wilkes Barre and Embassy of Scranton, Scranton, PA.

Job Duties

This position is as a Geisinger provider in a non-Geisinger facility and requires an experienced advanced practitioner, prior experience working in a skilled nursing facility, family medicine, internal medicine or ICU. The advanced practitioner (AP) provides direct (hands-on) care in skilled nursing facilities with a goal to improve quality and prevent unnecessary re-admissions to the acute care setting.

Responsibilities:

Review reportsCommunicate with SNF staff for patient needsTreat patients for acute needs, admissions, discharges, etc.Manage extremely complex patients, see every day or at least once a weekApproximately 12 patients a dayLength of time per patient and number of patients is per need
