Nurse Practitioner / Physician Assistant | Value Based Care - Quality and Wellness | Day | Full-Time
Overview This position is fully in the ambulatory or outpatient setting, with no inpatient responsibilities. The clinician will play an essential role on a multidisciplinary team, assisting in the delivery of care for patients undergoing care transitions (e.g., from the emergency department or inpatient stay to home), managing follow-up for chronic diseases, and closing health maintenance gaps. The role involves providing comprehensive diagnostic and therapeutic health care services to patients, including but not limited to: taking medical histories, performing medication reconciliation, conducting physical examinations, ordering and coordinating lab tests and diagnostic studies, prescribing medications, and guiding patients on treatment plans and health education. Specific responsibilities include conducting post-discharge or transitional care management visits, Medicare Annual Wellness Visits, and chronic disease management visits. This work is conducted under the direction of UFHP and primary care leadership and serves as a direct extension of the patient’s medical home, in partnership with their primary care provider. Patient interactions will primarily occur through telemedicine visits and telephone outreach, requiring strong electronic medical record (EMR) proficiency and excellent telemedicine communication skills with both patients and team members. Qualifications Minimum Education and Experience Requirements: Master's degree in Nursing or Physician Assistant. Credentialing: Must meet requirements set forth by the Medical Staff Office to obtain and retain privileges to practice as an Advanced Practice Registered Nurse or Physician Assistant within UF Health Shands Hospital and/or the Shands UF Healthcare Network, as applicable. Additional Requirements Specific Area or Specialty --/-- Additional Requirements: Adult ICUs --/-- AGACNP required. ICU Experience preferred. Emergency Medicine --/-- AGACNP required. Emergency Medicine Experience preferred. Pediatric ICU --/-- PNP-AC required. Neonatal ICU --/-- NNP or PNP-AC required. NICU experience preferred. Surgical/Operative Adult --/-- APRN with RNFA required. Adult Clinic/Primary Care --/-- AGACNP, AGPCNP, or FNP required. Pediatric Clinic/Primary Care --/-- PNP or FNP required. Nurse Midwife --/-- CNM or APRN with Florida license to practice midwifery required. Psychology/Psychiatry --/-- PMHNP required. Motor Vehicle Operator Designation: Employees in this position: Will not operate vehicles for an assigned business purpose NOTE: A frequent driver is defined as one who uses his/her personal or Shands automobile a) at least once daily, b) at least five individual trips per week or c) drives, on average, over 150 miles per week in the performance of his/her job. Licensure/Certification/Registration: Current and clear license as an Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) in the State of Florida. BLS is required.
