ACCURATELY monitors patients’ vital signs, intake and output, according to procedure. Performs blood glucose monitoring as delegated by and under the supervision of the Registered Nurse. Administers AM and/or PM care as appropriate. Provides general comfort measures to patients throughout the shift.
Makes rounds on patients, anticipates their needs, and answers/responds to call lights in a timely manner. Provides postmortem care. Safely assists patient in mobility functions, transfers, transports and discharges, identifying physical limitations unique to the patient population. Prepares patients for meals, assists or feeds patients, collects trays and records dietary intake. Reports mechanical or swallowing difficulties of the patient to the Registered Nurse. Distributes bedside nutritional supplements to patients. Collects and transports specimens as delegated by the Registered Nurse.
Assists other providers of the multi-disciplinary care team in providing CONTINUUM OF CARE. Reports any patient changes in condition or behavior to Registered Nurse. Provides input to patients plan of care. Assists with pre- and post-operative care such as preparing bed, positioning and vital signs. Handles all patient belongings from admission to discharge in a safe manner. Promotes a restraint free environment. Demonstrates knowledge related to applying and releasing safety devices/restraints under supervision of the Registered Nurse. Maintains knowledge and maintenance of equipment. Establishes and maintains an effective rapport with patient, family, nursing personnel, physicians, and personnel from other departments. Recognizes and responds to priorities in the patient care PROCESS.
