**General Summary/Responsibilities:**
The Nursing
Assistant Specialist is a paraprofessional member of the health care team who
provides physical and mental support for the patient, performs light
housekeeping and meal preparation and provides companionship and incidental
personal care. This specialist may work
under the supervision of a licensed nurse to perform basic nursing tasks as
assigned.

Assigned
to cases by Scheduling Department to work with patients in variety of settings.
**Organizational Standards**
§ Maintains
and supports overall philosophy, goals and objectives of Fidelity Health Care and
the Independent Care Program and its contracts.
§ Complies
with all agency regulations/policies/procedures:
o Submits all documentation timely per policy.
o Implements home care services according to plan of
care.
o Follows agency and governing bodies' standards of
practice.
o Communicates all pertinent clinical and
psychosocial information to supervisor and/or Independent Care team members as
indicated.
§ Attends
all scheduled in-services and staff meetings.
§ Appropriately
arranges time off coverage and communicates this with agency.
§ Is responsible for maintaining minimum weekend
on-call, holiday or other requirements established by the department.
§ Maintains
minimum productivity expectations.
**Job Limitations:**
· May not receive orders from a physician.
· May not administer medications or pre-fill
insulin syringes.
· May not perform any sterile procedures or
perform any intravenous procedures,
enteral feeding or give enemas.
· May not perform procedures requiring the
knowledge, training and skill of a
licensed nurse, as defined by the
supervisor/organization.
**EssentialDuties & Functions:**
1. Must be
flexible to work within hospital and home environment based on department
needs.
2. Assists
PCT and RN with care of the patient assigned
3. Demonstrates
knowledge of skin care including prevention, monitoring and reporting.
4. Performs
basic daily duties to manage the patient’s household,
i.e. personal needs, housekeeping, shopping,
meal planning and preparation,
laundry, errands, etc. Maintains a neat and orderly patient environment.
5. Assists
with incidental personal care as needed.
6. Provides
companionship by reading, talking, listening and interacting with patients and/or
caregivers. Utilizes appropriate
communication skills with the patient,
the patient’s family and all agency/facility personnel.
7. May take
home care patient shopping, on short
trips or on walks as necessary.
8. Follows
all agency policies regarding infection control and employee safety.
9. Completes
accurate records online or on paper, and carries out all assignments as
requested.
10. Demonstrates
respect for patient property and maintains patient confidentiality.
11. Recognizes
emergencies and demonstrates knowledge of emergency procedures.
12. Demonstrates
reliability and is able to arrive on time to patient assignments, dressed and
groomed in a professional manner.
13. Understands
safety and infection control.
**Education**
**Minimum Level ofEducation Required:**
High School completion / GED
**Licensure/Certification/Registration**
STNA or CNA
certification required
**Experience**
**MinimumLevel of Experience Required:**
1 - 3 years of job-related experience
**Knowledge/Skills**
· Must be able to read,
write, and follow written instructions
and document care given.
· Must have completed a Nurse Aide program and
have a CNA or STNA
· Must complete agency competencies.
· Must display good physical and emotional
health and be capable of stooping and bending and lifting up to 50 lbs. Should demonstrate good personal hygiene.
· Must be self-directed with the ability to work
with little direct supervision and have the maturity necessary for
establishing and maintaining a good work relationship with the patient, the patient's family and all other office
personnel.
· Must have a sincere interest in and
sympathetic attitude toward caring for the sick and disabled at home.
· Must meet all agency personnel policies.
· Must have reliable transportation.
