This is a full time position working days, and weekends as needed.

Main Function: The Nursing Home Placement Coordinator provides a centralized resource for all Nursing Home referral and discharge activities.  The BSW/BS role is essential for the communication and planning for post discharge care in a Nursing Home setting.  Excellent communication with unit interdisciplinary teams and Nursing Home Admissions teams is critical to our hospital throughput.

Education:      

Required – Bachelor’s Degree Preferred - BSW

Experience:

Required – no previous experience required Preferred – at least 2-3 years in a health care setting, preferably either an acute hospital setting or skilled nursing facility setting

Pay Range: $23.50 - $37.00

Starting Pay: Based on experience
