+ POSITION SUMMARY + Reports To: + Manager, Practice Operations, DCFM + Summary: + The OB Navigatorprovides family-centered, strength-based case management/care coordination (CM/CC) to facilitate access to medical care and community-based resources through screening and uptake through multiple modalities, including in-home, HS site, and/or virtually, to meet the unique needs of participants including high-touch support when indicated. Conducts outreach, recruitment, and retention efforts for health service participants. + Work Schedule: + Must be able to work non-standard hours and may be in excess of 40 hours per week. + SKILLS & ABILITIES + Education + Registered Nurse (RN), preferably with a Bachelor of Science in Nursing degree (BSN). + Experience + One year Healthcare, Community, or Public Health. 3 years of experience in labor/delivery or maternal/child health. + Computer Skills + Proficient in Microsoft Office, email and basic computer skills for other software used throughout the hospital for your area of responsibility. + Email and basic computer skills. Must be proficient in Microsoft applications. Knowledge in using electronic medical records and other databases. + Certificates & Licenses + Registered Nurse (RN) license in state of Indiana. + Other Requirements + Knowledge of changes in regulations and requirements. Knowledge of healthcare delivery system, medical terminology, screening material, assessment skills, patient safety and quality initiatives, management and treatment protocols and regulatory guidelines.
