Job Summary
The OB Navigator serves as a dedicated advocate for expectant mothers, ensuring they receive the education, care, and support needed throughout their pregnancy and postpartum journey. This role includes coordinating prenatal care, addressing patient concerns, and connecting patients with necessary resources. The OB Navigator supports patients from early pregnancy through 1 year postpartum, facilitating a seamless healthcare experience and promoting maternal and infant health.
Responsibilities
Patient Support:Serve as the primary point of contact for pregnant patients, providing guidance and support from the first prenatal visit through postpartum.Educates patients on prenatal care, labor and delivery options, postpartum care, and newborn care, ensuring patients understand their care plans.Address patient concerns, questions, and barriers to care in a timely and compassionate manner. Care Coordination:Maintains collaborative relationships with physicians, midwives, pediatricians, nurses, and other healthcare providers to ensure a holistic, team-based approach to patient care.Assist in the identification and management of high-risk pregnancies, ensuring appropriate referrals and resources are available. Resource Connection:Link patients to necessary resources such as nutrition services, mental health care, social services, maternal opioid services, in home services and breastfeeding support.Facilitate enrollment in programs such as Medicaid, WIC, and other local support services as needed. Education and Outreach:Provide educational materials and classes on prenatal and postpartum care, infant safety, breastfeeding, and other relevant topics. Follow-Up Care:Provide ongoing postpartum follow-up to ensure proper recovery, addressing mental health, breastfeeding support, and infant care.
Work Environment:
The OB Navigator may work in a clinical setting, hospital, or office setting. There may be an occasional evening or weekend work requirement for patient education classes or outreach events.Physical Requirements:
Ability to sit, stand, and walk for extended periods.Ability to lift and carry up to 20 pounds
