The Outpatient Addictions Program helps people get sober and maintain recovery. We offer medication management, along with individual and group counseling

Summary:
• In collaboration with other members of the healthcare team, the Case Manager is responsible and accountable for patient trajectory by assisting the healthcare team in pacing care toward improved patient outcomes.

Responsibilities:
• Completes an initial screen of all patients within 24 hours of admission utilizing specific criteria to identify needs related to utilization management and/or discharge planning. Refers appropriate cases to Social Work based on established referral criteria and professional judgment.
• Introduces self to the patient/family, explains the Case Manager role, and provides them with a business card.
• Integrates the discharge planning/coordination and utilization review functions in the case management role.
• Refers appropriate cases to Social Work based on established referral criteria and professional judgment.
• Collaborates with all members of the multidisciplinary team to facilitate the case management process for the assigned caseload.

Qualifications:
Licensure: MA RN Licensure required.
Certifications: Required to obtain addiction certification in nursing (CARN) within 2 years. AHA or ARC BLS certification required. 
Education/Training: Graduation from an accredited school of nursing. 
Work Experience and Skills: Minimum of 3 years nursing experience; current experience working with patients with opioid use disorder and/or other alcohol/substance use disorders is required. Demonstrated team leadership with the ability to work collaboratively with others. Demonstrated commitment to the principles of recovery, rehabilitation, and harm reduction. Demonstrated excellent verbal and written communications skills. Experience working with data; knowledge of the patient medical record system, EPIC, a plus. Able to make independent decisions, assess, organize, prioritize and address multiple projects and problems concurrently. Able to interact and collaborate successfully with patients, community members, network management and professional staff.Linguistic capabilities: Knowledge of a second language, preferably Portuguese or Spanish, would be seen as an advantage.
