Under general administrative direction, the OB Nurse Navigator shall be an entrepreneurial, enterprising, and inspirational leader with a strong passion for obstetrics, serving as the primary point of contact for OB patients throughout the prenatal, perinatal, and postnatal periods. This role is responsible for comprehensive care coordination, patient education, and seamless communication between multiple care areas. The OB Nurse Navigator will ensure that all aspects of patient care – from scheduling prenatal visits and coordinating labs and ultrasounds to scheduling C-sections and managing triage calls and messages. The Nurse Navigator collaborates with providers and/or clinical areas to coordinate services and ensure quality of care for patients. Assess the physical and psycho-social needs of patients and their families throughout their OB journey. This positon requires skill in physical examination skills, triage, adept use of technology, and great collaboration skills. Must be able to prioritize workload and respond to calls and messages in a timely manner. Demonstrates knowledge of the nursing process as a standard in accordance with licensure, incorporating elements of holistic family-centered care. All staff is considered to be an employee of St. Peter’s Health Medical Group first. Because of this, there is an expectation that they may float to other SPHMG areas when not needed in assigned area.
