Job Summary
The SBHU Occupational Therapist plans and conducts scheduled therapy group sessions/ programs, and individual sessions as needed on the Geropsychiatric Unit. The SBHU Occupational Therapist will participate in co-treatment activities with other disciplines as identified. The SBHU Occupational Therapist is responsible for completing assessment data, care planning and routine documentation. The SBHU Occupational Therapist participate as an integral part of the interdisciplinary treatment team on all matters concerning the patient.
Essential Duties and Responsibilities
+ Supports and promotes the mission and values of Covenant Health Ministry.
+ Creates/maintains a therapeutic environment
+ Insures facility is safe and reports deficiencies to supervisor.
+ Communicates results and trends to supervisor.
+ Provides feedback to supervisor on ongoing basis in regards to concerns, improvements, changes, etc.
+ Identifies areas needing improvement and utilizes the facility performance improvement process. Actively participates in comprehensive program evaluation system.
+ Utilizes universal precautions at all times to prevent and control infection.
+ Ensures confidentiality of all confidential information encountered.
+ Completes routine paperwork within the prescribed time frames and efficiently documents on appropriate forms all required, pertinent and meaningful observations and information.
+ Can participates in departmental Process Improvement activities
+ Completes all required competencies per program regulations.
+ Keeps current on CEU requirements to maintain licensure status.
+ Attends in services, seminars, etc. to promote learning.
+ Other duties as consistent with this role.
Job Requirements: (education and experience/preferred and required)
Job Knowledge and Skills
+ Assesses the patient’s needs, abilities, and barriers following the patient’s entry into the program
+ Obtains information related to the patient’s previous activity engagement and interests, and functional ability to continue to participate in those activities, and abilities to participate back in the community.
+ Evaluates patient’s ability and desire to participate in new leisure activities based on new disabilities.
+ In the presence of newly acquired disabilities, evaluates the patient’s abilities and potential limitations for participation.
+ Participates consistently in team meetings, reporting the results of assessments, interventions, and planning for the continued treatment and discharge of patients and families.
+ Participates in treatment planning process, including discharge planning on those patients who will benefit from occupational therapy/therapeutic recreation interventions.
+ Plans and conducts scheduled group sessions/programs, and individual sessions as needed in accordance with the interdisciplinary treatment plan and daily priorities of patients and needs of the unit.
+ Adapts group and individual activities and goals to meet the needs of the patients.
+ Assists with orientation of patients and/or families to the unit and the program.
+ Reports to charge nurse all pertinent patient/unit observations/actions in order to ensure continuity of care.
+ Demonstrates consideration of resource management.
+ Provides resources and support for patients before, during, and after treatment of patient.
+ Participates in discharge/aftercare planning by working with the interdisciplinary team to identify and meet patient and/or family needs as documented in the patient care plan.
+ Participates in and/or facilitates community meetings, activity groups, and patient education groups.
+ Demonstrates ability to effectively prioritize patients during high volume.
+ Communicates evaluation results and treatment plan to MD, rehab team, patient and family
+ Demonstrates effective time management by using time between patients for documentation, filing, special projects
+ Documentation includes measurable and functional long term and short-term goals
+ Adjusts daily treatment goals based on changes in patient status
+ Develops protocols/groups for appropriate diagnosis to enhance quality of patient treatment
Education and Experience
+ NH Occupational Therapy License required
+ Crisis Prevention Certification (CPI) required
+ Basic Cardiac Life Support certification required
+ Bachelor’s degree in Occupational Therapy preferred
Working Conditions/Physical Demands
Must p ossess the physical and mental abilities to perform the tasks normally associated with this position that involves a combination of sitting, standing, walking, bending, stooping, and reaching. Lifting/carrying frequently up to 35 pounds, and occasionally up to 50 pounds. Some stress related to high level of responsibility for quality care.
