You will be:
To carry out Care Act Assessments for those discharged from hospitals.Work with other professionals to ensure the best outcome for Service Users.Completing CHC Assessments, Mental Capacity Assessments and Functional Assessments.Essentials:
Valid Right to Work in the UK.Registered to HCPC.Industry relevant Frontline Experience.Up to date with industry knowledge and legislation.Subject to a clear enhanced DBS check.Why Vox?
