Responsibilities Evaluate patient’s functional status and home environmentIdentify equipment needs and participate in the development of the total plan of careDevelop treatment program and establish goals for improved functionTeach new skills, retrain in once familiar daily activity skills, fabricate splints, train in use of prosthetic and/or orthotic devicesMaintain appropriate clinical records and communication with other personnel involved in patient’s careSupervise Occupational Therapy Assistants according to organization policy and state regulationsParticipate in discharge planningParticipate in case conference as requiredEnsure compliance with all state and federal legal and regulatory requirementsAdditional responsibilities as assigned Qualifications Graduate of an occupational therapist education program accredited by ACOTE with current licensure to practice in state of operation2+ years occupational therapy experienceHome health experience preferredCommitment to clinical and documentation excellence About our Line of Business At At-Heart Home Care & Hospice, which achieved a 4.5 quality rating on CMS Home Health Compare, our primary ambition is to bring first-class care to the warmest and most serene environment possible: home.
