The Occupational Therapist – Home Health Care/Hospice plans, interprets, and implements specific occupational therapy treatment programs for patients on the Home Health Care/Hospice caseload. This individual is responsible for accurate record keeping, documentation of the duties completed and notification to the Registered Nurse Case Manager, Registered Nurse Clinical Supervisor, or appropriate team member of any change in status observed on a visit to a patient.
The Occupational Therapist – Home Health Care/Hospice will select methods or strategies to direct the process of interventions, evaluate factors affecting activities of daily living and implements interventions and procedures to promote or enhance safety and performance in activities of daily living. The Occupational Therapist – Home Health Care/Hospice performs in a manner consistent with Marshfield Clinic Health Systems’ mission, policies and initiatives while maintaining patient confidentiality at all times.
JOB QUALIFICATIONS
EDUCATION
For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.
Minimum Required: Bachelor’s degree in occupational therapy.
Preferred/Optional: Master’s or Doctorate degree in occupational therapy.
EXPERIENCE
Minimum Required: Supervised clinical internship(s) working with adult and/or pediatric populations to prepare for professional registration.

CERTIFICATIONS/LICENSES
The following licensure(s), certification(s), registration(s), etc., are required for this position. Licenses with restrictions are subject to review to determine if restrictions are substantially related to the position.
Minimum Required:
Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) within three months of hire.Occupational Therapist license awarded by the State of Wisconsin.Preferred/Optional:
