Join the Cleveland Clinic team, where you will work alongside passionate and dedicated caregivers, receive endless support and appreciation, and build a rewarding career with one of the most respected healthcare organizations in the world. **A caregiver in this position works on a PRN-as needed basis, 8am-4:30pm weekday and weekend coverage needed!** A caregiver who excels in this role, will: + Evaluate and treat patients with physical, emotional, cognitive and/or psychosocial deficits which impact their abilities to successfully perform independent living skills. + Perform initial and ongoing assessments of patient's condition. + Establish, revise, and evaluate a plan of care which is appropriate to problems identified and involves the patient/family according to their capabilities and desires. + Perform therapy interventions utilizing standard occupational therapy techniques and skills as appropriate for the age and condition of the patient. + Supervise activities of occupational therapist assistants, students, and other support personnel. + Always maintain complete and up-to-date documentation and files regarding patient condition and effectiveness and timeliness of care and interventions. + Participate in ongoing interdisciplinary assessment of assigned patients. Participates as a member of the hospice interdisciplinary team when necessary. Minimum qualifications for the ideal caregiver include: + Bachelor's Degree required. + Must be a graduate of an Occupational Therapy Program accredited by the American Occupational Therapy Association. + Current State Licensure as an Occupational Therapist (OT). + Current Basic Life Support (BLS) certification through the American Heart Association (AHA).
