Occupational Therapist (OT) - Home Health

Healthy Living at Home is seeking a dedicated and compassionate Occupational Therapist (OT) to join our team in Modesto, CA. The OT will be responsible for implementing standards of care for occupational therapy services, ensuring adherence to all conditions outlined in the Service Agreement. This role is pivotal in helping our patients achieve improved functional status and independence in their homes.
Compensation starting at $65/hour, DOE, Open to Negotiation!
Key Responsibilities:
Patient Evaluation: Assess patients’ functional status, including muscle function, endurance, visual coordination, communication skills, self-care ability, and work capacity. Evaluate home environments for potential hazards or barriers to independent living. Identify necessary equipment.Care Planning: Develop individualized treatment programs and establish goals for improved function. Communicate plans of care to referring physicians and other professionals.Skill Training: Teach new skills or retrain patients in daily activities affected by illness or injury. Fabricate splints and instruct patients on the use of adaptive equipment.Prosthetic/Orthotic Training: Train patients in the use of prosthetic and orthotic devices.Record Keeping: Maintain clinical records, notes, and report any changes in patients’ conditions to physicians. Submit records per organizational policy.Team Coordination: Maintain communication with all personnel involved in patient care to ensure coordinated and efficient care. Attend in-services, case conferences, and other required meetings.Family/Caregiver Education: Instruct patients’ families/caregivers and other healthcare personnel in treatment regimes when necessary.
Qualifications:
Graduate of an accredited occupational therapy curriculum.Registered with the National Registration Examination of the American Occupational Therapy Association.Minimum of two years of clinical experience as an occupational therapist.Strong verbal and written communication, and organizational skills.Current BLS Hands-On CPR Certification.
